My second worst is the out of order sign I put on the the first floor entrance to an elevator in a twelve story building. I then rode the elevator to the 12th floor where I worked and settled in to watch the stairwell door open and see many of my co-workers crawl in...breathing heavy...and then learn from others that the elevator did indeed work.
